INDIANA HOME AND COMMUNITY BASED WAIVER CASE MANAGEMENT

Person Centered Case Management is a comprehensive approach to promote health and safety in a community-based setting through continuity and quality of services. AIHS Case Managers empower living in settings-of-choice by delaying nursing home admissions, and keeping individuals independent and supported in the community, for as long as possible. This is demonstrated through collaborative and integrated efforts for all services developed in your personal care plan.

Update Regarding Indiana Home & Community Based Services Waiver

In April 2026, AIHS was approved by the State of Indiana to provide services under the Medicaid Home and Community-based Services (HCBS) Waiver. We are now offering services to individuals enrolled in the following Medicaid Waivers:

  • Community Integration and Habilitation

  • Family Supports Waiver

  • Health and Wellness

  • Traumatic Brain Injury

  • PathWays Fee for Service

  • Money Follows the Person (PathWays, Health and Wellness, and CIH)

Eligibility to Enroll:

There is no cost to participate in the Case Management program. However, each program will have its own set of eligibility requirements – Contact the Aging & Disability Resource Center (ADRC) at 260.469.3036 to request more information.
