Providing care for an aging loved one can be stressful, especially when the care recipient has one or more chronic conditions. Between managing appointments and making sure they are safe at home, the job of the caregiver can feel overwhelming.
Programs of All-Inclusive Care for the Elderly, also known as PACE, is a Medicare and Medicaid funded program that can help lighten the caregiving load. Through a strategic partnership between Aging & In-Home Services of Northeast Indiana and Parkview Health, PACE of Northeast Indiana offers both clinical care and social services in one setting.
PACE Services Include:
PACE services are provided at the discretion of the Interdisciplinary Team (IDTeam). This team of professionals is responsible for laying the groundwork of a Participant’s care through the creation of their individualized care plan. Made up of 11 disciplines, the diversity and expertise of the IDTeam ensures that all needs of the Participant are being met with an all-inclusive, integrated and centralized plan.
Services offered through PACE of Northeast Indiana may be available at low or no cost. Currently, 90% of PACE Participants are dually eligible for Medicare and Medicaid. 9% are eligible for Medicaid only, and the remaining one percent pay a premium (National PACE Association, 2019). Private pay is available for those who do not qualify for Medicare and Medicaid.
Individuals interested in becoming a Participant must meet four eligibility requirements:
PACE of Northeast Indiana Eligibility Requirements:
If you believe you (or your care recipient) meet these requirements, consider starting the PACE enrollment process. Depending on an enrollee’s Medicare/Medicaid status, this four-step process takes about 30-45 days. Those who complete the enrollment process will begin receiving services on the first day of the following month.
