GERIATRIC & DISABILITY CASE MANAGEMENT
Geriatric & Disability Case Management is a comprehensive approach to promote health and safety in a community-based setting through continuity and quality of services. AIHS Case Managers empower aging-in-place by delaying nursing home admissions, keeping individuals independent in their own homes, for as long as possible. This is demonstrated through collaborative and integrated efforts for all services developed in your personal care plan.

What services can I receive from
Geriatric & Disability Case
Management?

Case Managers will perform a thorough
assessment to understand your needs,
customize care plans, and arrange services to
meet your needs. Once services and
arrangements are in place, your Case Manager
will begin monitoring your progress. Your Case
Manager is also available to you, at any time, to
answer questions that may arise.

Are there eligibility requirements or
costs to participate?
​
There is no cost to participate in the Geriatric &
Disability Case Management program.
However, each program will have its own set of
eligibility requirements – Medicaid Waiver,
Community and Home Options for Institutional
Care for the Elderly and Disabled (CHOICE),
Social Services Block Grant (SSBG), Title III.
