GUIDE SUPPORT SPECIALIST

Summary/Objective

Support the administrative and clinical functions of the GUIDE Program under the direction of the Clinic Manager. Administrative functions are client-facing at front desk, registration, up to and including backroom processing of files in selected software systems. Clinical functions are to support the clinical procedures performed by the Nurse Practitioner and include, among others, vitals, urine samples and medication review.

 

Essential Duties/Responsibilities

  • Provide administrative coverage of front lobby including greeting visitors in a professional and welcoming manner, accessing paperwork to determine status as a referral or patient, directing to appropriate staff, i.e. patient room for clinical visit, office for care navigation education, etc,

  • Point person on all calls, emails, faxes, etc and respond in a timely manner to all including referring to appropriate person for timely response.

  • Respond/initiate patient record requests as needed.

  • Process GUIDE referrals including management of master referral list as new referrals as received, completion of screening calls per protocol, and verification of eligibility

  • Manage patient appointments including scheduling assessment in Epic, sending enrollment paperwork to scheduled patients, and managing contact to ensure appointment is kept and to ensure patient/caregiver are aware of upcoming appointment and have completed required paperwork.

  • Clinic duties include patient registration, rooming patients and caregivers, review and update of patient information, taking vital signs and documenting in EMR, conducting specimen collection as directed, completing medication review, and conducting initial patient enrollment assessment as directed.

  • Packets/documents- ensure clinic has adequate documents on hand and collate new packets as needed (i.e. Enrollment packets, Caregiver Handbooks, Patient Education Handouts, and Marketing materials).

  • Manage mailings to patients such as enrollment packets and satisfaction surveys.

  • Conduct patient education as directed.

  • Identify patient’s needs and escalate all emergencies/concerns to the GUIDE care team immediately.

  • Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies.

  • Serve as an example of agency Standard of Excellence by exemplifying agency vision and goals.

  • Communicate with supervisor on an immediate/weekly basis, as needed, to report all areas of concern/issues/incidents.

  • Demonstrate professionalism in relationships with customer/co-workers/ supervisors to build rapport while refraining from inappropriate familiar personal relationships.

  • Complete and attest to accuracy of payroll records and submit for review and processing according to agency schedule.

Required Education and Experience

  • High School Diploma/GED.

  • Certification/ Licensure as Medical Assistant or LPN

  • Or minimum of 2 years’ experience in a healthcare setting in lieu of MA/LPN

 

Preferred Experience and Education

  • Current license as LPN.

  • Two years of related experience in a healthcare setting.

  • Experience working with older adults.

  • Experience working with Persons Living with Demetia (PLWD).

 

Additional Eligibility and Requirements

Must possess and provide annually valid driver’s license and documentation of current automobile insurance at agency-specified levels. Must provide negative TB certificate, or documentation to support exemption, annually. Must comply with requirements of other facilities (hospitals, LTC facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities. 

 

Expected Hours of Work

This position is non-exempt. Agency business hours are typically Monday through Friday, 8:00am-4:30pm.   

 

Physical Demands

While performing the duties of this job, an employee is regularly required to:

  • Have the ability of close vision.

  • Speak and hear.

  • Stand, walk and sit.

  • Use hands to finger, handle or feel objects, tools or controls.

  • Reach with hands and arms.

  • Stoop, kneel, crouch or crawl.

  • Bend down and lift objects from the floor.

  • Lift up to 25 pounds and carry it 15 feet.

 

The employee may be occasionally required to:

  • Lift or move up to 50 pounds and carry it 15 feet.

  • Climb at least 3 flights of stairs (ascend and descend)

  • Balance

  • Push 300 pounds in a wheelchair.

 

The individual must be able to communicate effectively with others and enable understanding by others through communication. The individual is required to sit and/or stand for long periods of time.

 

Work Environment

This job is operated in a professional office / clinic environment using standard office equipment such as computers, phones, photocopiers, fax machines, etc and in a standard clinic setting performing clinical procedures as trained within licensure as Certified Medical Assistant or LPN (Licensed Practical Nurse).
