DIETITIAN
Summary/Objective
Optimizes nutritional status of PACE participants through comprehensive nutrition assessment, monitoring, and communicating concerns & recommendations to the PACE Interdisciplinary Team. Provide comprehensive nutrition assessments and counseling to patients at the AIHS GUIDE clinic.

Essential Duties/Responsibilities
-
Perform nutritional assessments upon enrollment and re-assess every 6 months or as needed for significant changes in clinical status. Record periodic nutritional care data in the medical record. Review and adjust therapeutic diets to reflect any changes in the daily menu and suggest the use of supplemental feedings when necessary. Provide diet instruction to participants and/or family members on prescribed diets.

Evaluate and counsel assigned clients about health and dietary needs. Prepare written reports and document case notes within established timeframes.

Develop and/or acquire evidence-based health and wellness programming suitable for older adults.

Maintain Registered Dietician license, including continuing education in diabetes related education, according to State requirements.

Provide Diabetes Self-Management (DSM) education and training to assigned clients.

Support agency accreditation in Diabetes Self-Management Training.

Work with Community Project Specialist, Research Department staff and Nutrition Department staff to plan, implement and maintain wellness programs.

Work with Community Project Specialist and Research Department staff to recruit and train staff, volunteers, and potential research subjects to assist with or participate in wellness activities and wellness-related research.

Partner with academic programs or researchers as appropriate to develop and provide wellness opportunities to older adults, individuals with disabilities, their caregivers, community members and other community-based providers.

Coordinate and/or participate in community education events related to health and wellness.

Assist in providing administrative direction for meal service; establish programs, policies and procedures that will enforce safe food handling practices.

Coordinates both inter and intra departmental professional activities to provide optimum nutritional care for the participants and serve as consultant to clinical staff on matters pertaining to dietetics and nutrition. Attend and participate in daily Interdisciplinary Team (IDT) Meetings.

Assist in development and delivery of departmental and facility-wide educational programs.

Maintain rapport with participants, visitors, and family members and provide feedback on the overall PACE participant attitude about food service.

Provide Medical Nutrition Therapy as ordered by medical provider.

Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies.

Organize and document daily schedule to inform supervisor of activities and adhere to safety protocol.

Demonstrate competency by meeting or exceeding department work performance requirements.

Communicate with supervisor on daily basis, and as needed, to report all areas of concern/issues/incidents.

Serve as an example of agency Standard of Excellence by exemplifying agency vision and goals.

Demonstrate professionalism in relationships with customers/co-workers/supervisors to build rapport while refraining from inappropriately familiar personal relationships.

Complete and attest to accuracy of payroll records and submit for review and processing according to agency schedule.

Required Education and Experience
Must be a registered dietician with a bachelor’s degree. Must have one year experience working with the frail and/or elderly. Able to teach, think analytically. and relate to geriatric patients. Knowledge of nutrition care process and federal, state, and local standards and regulations.

Preferred Education and Experience
Master’s degree in nutrition.
Additional Eligibility Requirements
Must possess and provide annually valid driver’s license and documentation of current automobile insurance at agency-specified levels. Must provide negative TB certificate, or documentation to support exemption, annually. Must comply with requirements of other facilities (hospitals, LTC facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities.
Expected Hours of Work
This position is a non-exempt position and may be full-time or part-time depending on the agency’s needs. Agency business hours are typically Monday through Friday, 8:00am-4:30pm.

Travel
Travel is throughout the agency’s service area during agency business hours.
​
Physical Demands
The individual must be able to communicate effectively with others and enable understanding by others through communication. The individual is required to sit and/or stand for long periods of time. The individual is required to transport, safeguard, and use a laptop computer and client files.

Work Environment
This job operates in a professional office environment using standard office equipment such as computers, phones, photocopiers, fax machines, etc. This job also operates at clients’ residences.
