CASE MANAGEMENT ASSISTANT

Performs all activities related to support of the Case Management Department. Responsible for performing clerical, communications, and follow-up activities for and with staff, individuals/families served, and providers as assigned in a confidential and timely manner.

Essential Duties/Responsibilities

  • Support Case Management Department by providing clerical & administrative support, such as:

    • Mailing, faxing, scanning items as needed for agency.

    • Research resources and complete referral forms

    • Facilitate communication with Individual/Families served via phone and in person

    • Assist with Medicaid applications

    • Prepare electronic forms as needed

    • Process required documents according to program guidelines

  • Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies and report any potential issues immediately 

  • Develop and maintain an accurate and thorough knowledge of all program requirements

  • Develop and maintain skillset on tools required to perform job tasks in timely and accurate manner including specialized computer software

  • Develop and maintain an accurate and thorough knowledge of hardware platforms and software applications used to communicate effectively with Individuals served. Must be able to effectively use telehealth/technology implemented for virtual communication.  

  • May periodically serve as part of front desk rotation responsible for greeting visitors in a welcoming manner and efficiently routing to appropriate staff during normal business hours.

  • Demonstrate competency by meeting or exceeding department work performance requirements 

  • Communicate with supervisor on daily basis, and as needed, to report all areas of concern/issues/incidents 

  • Serve as an example of agency Standard of Excellence by exemplifying vision and goals  

  • Complete and attest to accuracy of payroll records and submit for review and processing according to agency schedule.

  • Demonstrate professionalism in relationships with customers/co-workers/supervisors to build rapport while refraining from inappropriately familiar personal relationships

Required Education and Experience 
Associate’s degree from two-year College or technical school and two years of experience and/or additional education in human services or related field. High school diploma, or general education degree (GED) and four years of related experience and/or training in healthcare and/or customer service position utilizing advanced technology tools including customized software systems. 
       
Preferred Education and Experience
Related experience working with older adults. Experience in Healthcare, or Medicaid/Medicare setting. Experience working with state agencies and/or Medicaid approval process preferred. Demonstrated background in administrative support.

 

Additional Eligibility Requirements
Must possess and provide annually valid driver’s license and documentation of current automobile insurance at agency-specified levels. Must provide negative TB certificate, or documentation to support exemption, annually. Must comply with requirements of other facilities (hospitals, LTC facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities.  

 

Expected Hours of Work
This position is a full-time non-exempt position.  Agency business hours are typically Monday through Friday, 8:00am-4:30pm.  

 

Travel 
Travel is throughout the agency’s service area during agency business hours.

Physical Demands 
The individual must be able to communicate effectively with others and enable understanding by others through communication. The individual is required to sit and/or stand for long periods of time.  The individual is required to transport, safeguard, and use a laptop computer and client files. 

 

Work Environment 
This job operates in a professional office environment using standard office equipment such as computers, phones, photocopiers, fax machines, etc. This job also operates at clients’ residences.
