Conversation and moderated Q&A with internationally recognized expert on dementia and policy Mike Splaine, CEO, Splaine Consulting, Dementia and Alzheimer’s Advocacy Consulting, on the challenges in services to solo aging individuals with Alzheimer’s Disease.
Results of AIHS’ Hospital 2 Home two-year grant program funded by the Administration for Community Living to provide a 30-day evidence-based intervention; program participants had improved health outcomes, and less than a 5% hospital readmission rate.
