PREPARE for your Care: Step by Step Advance Care Planning

Advance Care Planning (ACP) is the process by which you pre-determine your wishes in regard to medical care and treatment should you be unable to communicate your wishes for yourself. Despite the importance of this planning, only a third of Americans have actually completed an advance directive – a form you must complete to put your wishes into writing (Health Affairs).

So, why have more than half of Americans failed to do any advance care planning? Not only can ACP seem confusing, but it can also be difficult to talk about the end of life.

Fortunately, there are several online resources that can assist you as you carry out your ACP. PREPARE for your Care is one such program that guides you step-by-step through the ACP process and completing your advance directives. Let’s take a look at the steps.

One of the many benefits of ACP is that it is extremely flexible. It’s up to you to decide how much (if any) you complete. Talking about the end of life isn’t easy, but planning ahead can help reduce stress for you and those around you.
