Advance Care Planning (ACP) is the process by which you pre-determine your wishes in regard to medical care and treatment should you be unable to communicate your wishes for yourself. Despite the importance of this planning, only a third of Americans have actually completed an advance directive – a form you must complete to put your wishes into writing (Health Affairs).


So, why have more than half of Americans failed to do any advance care planning? Not only can ACP seem confusing, but it can also be difficult to talk about the end of life.
