Case Management

Our Case Management team provides in-home assessments to authorize a number of funded services which delay or prevent inappropriate institutionalization. They work with vendors to ensure that quality services are provided to each individual.

Case Management services are available 24-hours a day.

Our Case Managers:
Assess the need for services
Screen for eligibility
Link to services and resources
Monitor to assure satisfaction
Adjust to meet changing needs
Advocate to best represent clients

As the single point of entry for individuals in need of funding assistance for long-term care services, we also have Case Managers completing Pre-Admission Screening for Nursing Facility placement.
