Our Case Management team provides in-home assessments to authorize a number of funded services which delay or prevent inappropriate institutionalization. They work with vendors to ensure that quality services are provided to each individual. Case Management services are available 24-hours a day.

As the single point of entry for individuals in need of funding assistance for long-term care services, we also have Case Managers completing Pre-Admission Screening for Nursing Facility placement.
